These are 12 month supported accommodation programs in which the young person is assisted to transition into independent living through one on one case management. Placement in the SHY/Caseworker Program involves regular home visits to assist the young person to develop life management skills. This is based on the young person’s needs with a case plan developed in consultation with the young person. This may include developing budgets, healthy meal planning, cooking lessons, attending gyms sessions, engaging in positive recreational activities, referral support to specialty services, support achieving educational/ career goals, advocacy, etc. Our staff provides the individual young person with consistent and compassionate holistic support as they navigate their transition to independent living or integrate into the community.
The SHY Program is designed to support young people exiting custody, at risk of entering custody, or assessed as needing supported housing. Referral to SHY is from Juvenile Justice ONLY.
The Caseworker Program is open for referral from community organisations, families or young people themselves. Completion of the Living Skills Program is a prerequisite to entering the Caseworker Program.
